You could also set this up on a very simple Drupal site using the webform module: http://drupal.org/project/webform

Students could be allowed to submit forms anonymously, or they could choose to include their name (if they wished).

Additionally, all submissions can be exported in CSV format, which would allow you to analyze the data in a variety of ways.

Do any of you use a program for students to do class/teacher evaluations?

We are looking for something simple that will compile the results by class, and will include not only ratings but comments.

The survey monkey we used had a hard time integrating comments with ratings and also separating out classes.
