Re: Permission for Photos, Films, Recordings and Student Work

Media Release

Publications, Video, Internet Consent and Release Agreement

Students who attend St. Raphael's Catholic School (herein after =20
referred to as SRS) the parish of St. Raphael's Catholic Church =20
(herein referred to as Parish) are occasionally part of publicity, =20
publications and/or public relations activities. In order to guarantee =20=

student privacy and ensure your agreement for your student to =20
participate, SRS asks that you agree to this form.

The form referenced below indicates approval for the student's =20
picture, art, written work, voice, verbal statements or portraits =20
(video or still) to appear in school publicity or parish publications, =20=

videos or on the SRS or Parish website. For example, pictures and =20
articles about school activities may appear in local newspapers, =20
school or parish publications. These pictures and articles may or may =20=

not personally identify the student. The pictures and/or videos may be =20=

used by SRS or the parish in subsequent years.

AGREEMENT

Student and Parent/Guardian release to SRS the student's name, =20
picture, art, written work, voice, verbal statements, portraits (video =20=

or still) and consent to their use by SRS and the parish.

SRS agrees that the student's name, picture, art, written work, voice, =20=

verbal statements, portraits (video or still) shall only be used for =20
public relations, public information, school or parish promotion, =20
publicity, and instruction.

Student and Parent/Guardian understand and agree that:

=B7 No monetary consideration shall be paid;

=B7 Consent and release have been given without coercion or =
duress;

=B7 This agreement is binding upon heirs and/or future legal =20
representatives;

=B7 The photo, video or student statements may be used in =20
subsequent years.

Choose 1:

o I have read this release and understand its contents


o I have elected to not allow any publicity of my student, written =20=

notification, in addition to this form, must be sent to the school =20
office.
