Permission for Photos, Films, Recordings and Student Work

How does your institution handle permission for using images of students
or student work in publications or on the web? Do you ask separate
permission to publish a student's name along with their photograph?

Since our current process is tremendously cumbersome, I'm recommending
we place a blanket statement in the contract such as the following: "I
grant permission for the school, in whole or in part, to use
photographs, written extractions, and voice recordings of students for
the purpose of marketing and publications unless otherwise notified in
writing."

How do I allay fears that parents will be upset by this? Can anyone
offer any suggestions?
