policy needed on substitute teachers

Hello we are reviewing and revising our policy concerning substitute
teachers. Any and all input or sample policies/applications appreciated.

1) do you require criminal check?
2) Application?
3) do you have a committee review?
4) what are basic requirements? ( College, High School Degree, prior
teaching exp?)
