We send parents/guardians a form entitled Missouri Military Academy's
Permission Questionnaire. It consists of 15 yes/no questions. One of the
questions asks if parents/guardians give permission for their son to attend
educational field trips, sometimes mixed with entertainment. We also
indicate that the cost to the school is often passed on to parents.

We're looking for blanket permission forms for field and class trips that
will take away the need to sign a form every time we take a class, group of
students or team off campus. Naturally, this does not include spring break
or summer trips.
