Course Evaluation System?

We want to move to a course evaluation/feedback system whereby student complete an online course-evaluation that is delivered directly to each participating teacher. The key is that there is complete anonymity and the questions can be adapted by each teacher.

One possibility is to train faculty on using something like Surveymonkey - but this is messy. Ideally I'd like a system so that a teacher can choose from a few of our own self-created templates, adapt it, and then issue it to their students.

Anyone have any ideas on such a course-evaluation system?
