We have used formstack.com and wufoo.com for similar applications - very
useful and easy to use.

On a more in-house level, if you have someone at your school who knows
Acrobat, both forms can be created easily. The Submit button can be set
to send information in a variety of formats to a specific person or to a
database set up on a server. Further, one could then use something like
Crystal Reports to generate reports from the data.
Large scale is probably best left to outsourcing.
