I would second that you don't want to go the adobe route. If you are using an Access DB you could hire a developer to quickly create some ASP/.Net pages to do this with some ease for $1000 or so.

Recently though I used a product from Caspio ($35/year base) that worked well

Like Zoho it allows you to create simple forms but allow you to do a lot more, including workflow, if you need it. Given that this is something simple that is likely to change over time it is probably worth investing in Zoho, Caspio or similar instead of having to re-develop if you do not have a programmer in-house.

I'd agree that the acrobat route may be a dead end, and a web form might be better.

For quick, ad-hoc web forms whose results you want to be able to download and tabulate, I recommend Zoho creator (creator.zoho.com). It lets you create embeddable web forms so you can just drop the forms into your own website with very little work. It's similar to google forms, but I find it's a bit more powerful. If you don't mind very subtle zoho branding, their free plan would probably suit this purpose.
