Policies on Parents and Technology Use in School

We have developed responsible use policies for our teachers, our upper and
middle school students, and are about to do the same for our lower school.
The policies for students require a parent signature indicating that the
parent has read and discussed the policy with their child.

We are looking at doing some sort of form, perhaps a code of conduct, for
our parents to point out our expectations of them relative to their
student's work and our communication system. As an example, if we begin
doing quizzes, assessments,etc through our content management system, we
want to make it clear that the work must be that of the student and not the
parent. We might compose a statement to the effect -- "I understand that all
quizzes submitted electronically must be the sole work of my child."

Question - has anyone developed a "responsible use" policy of sorts for
parents? If so, would you please share it?
