Here is a link to a sample of our admin. evaluation tool. This is a template that was adapted for each admin. with the aim to collect specific feedback for that person in their role in the school. Depending up on the position, the survey was sent to
25-30 people that included peers, faculty, the Head of School and in some cases, parents. They each made the changes and added role specific questions that I incorporated and then sent out to their list of evaluators. It seemed to work well when we did
