Survey Questions and Anonymity

With all the posts about surveys, I thought I would let you all know that my company just
started a promotion on a Satisfaction Survey Package
(http://bedrickconsulting.com/Surveys.php)
that creates, administers, tabulates results and generates interactive reports in HTML, PDF,
PPT and Word formats.

To Kris and all those asking about what questions to ask, the package includes a tool that
allows you to select from a list of commonly asked questions as well as add your own custom
questions. Even if you are not interested in the package, I would be happy to offer you some
free advice on how to go about selecting your questions.

As far as maintaining anonymity, we can administer the results as an independent 3rd party to
make sure that your management cannot trace the responses back to the responder. If anybody
would like to talk more about surveys, feel free to contact me offline.

The question regarding user anonymity, when accessing Surveymonkey, has been raised by concerned faculty worried that there could be a "digital trail" linking them to their responses. As a result we are exploring other avenues but I was wondering if this situation had arisen anywhere else and if so how it had been dealt with.
